How should White proceed?

The engines prefer 5.Qb3!?, continuing 5...Bxf3 6.gxf3 dxc4 7.Bxc4 Qd7 8.dxe5 Na5, when both 9.Bxf7+ and 9.e6 seem to give White an edge.
A major alternative is 5.cxd5 Qxd5 6.Nc3 Bb4, when the engines like 7.Be2!? exd4 8.0-0!? Bxc3 9.bxc3 d3!? with maybe a slight edge for White. If 7.Bd2, the engines give 7...Bxc3 8.bxc3 e4 9.c4 Qd6 10.h3 Qh6 11.hxg4 Qxh1 12.Ng5, when the bishop-pair gives White a lot of compensation for being the exchange down.
5...dxc4 6.Qxd8+ Rxd8 7.Bd2?!
7.Bxc4 Nxe5! 8.Be2 (8.Nxe5?? Rd1#) may give Black a tiny edge after both 7...Nxf3+ and 7...Nd3+, but is surely preferable to surrendering a pawn, which is what the text does.

The engines give 9.Ke2!?, albeit preferring Black. Also interesting is 9.f4!? Nd3+ 10.Ke2, the point being that after 10...Nxb2 (the engines reckon 10...h5 is best) White gets a strong initiative with 11.Bg2 c6 12.Ba5 Rd7 13.Nd2, when White has full compensation for being two pawns down, according to the engines.

This seems rather slow. The engines give 13.e4, but much prefer Black.

This is not really castling into it, despite possible appearances.

The d pawn's position may look precarious, but there seems no way for White to capture it.

White's play over the last few moves has seem fairly aimless, but it was not easy to come up with a constructive plan other than to wait and hope an opportunity arises when Black tries to make the extra pawn count.

Black retains a large advantage after this exchange sacrifice, but the engines reckon Black's advantage is even bigger after a more-patient move such as 27...f6.

Slow. Better is 31...Nf6, when 32.Bxf6 gxf6 33.Re7 Rxd6 34.Rxf7+ Kg6 is an easily winning rook-and-pawn ending for Black, according to the engines.

The engines point out 32.b3! cxb3 33.Kxd3, eg 33...f6 34.Bh2 a5 35.Kd4 (not 35.Re7+? Rxe7 36.dxe7 Ng7) Ng7 36.Kc5 with a large advantage for White. Black could try 32...Kg8, but then 33.bxc4 bxc4 34.Rc1 seems good for White.
Note that 31...Nf6, the reply 32.b3 is nowhere near as strong as in the game as 32...cxb3 33.Kxd3 Ne8 is very good for Black.
32...f5?!
Probably better is 32...Ng7, with ...Nf5 to come.
33.Re7+?
Black has at best a slight advantage after 33.b3! Kg7 34.bxc4 bxc4 35.Be5+, according to the engines. The text accedes to a very bad minor-piece ending.
